Covers fetch automatically and, once found, the actual image is saved into the file (not just linked) so it stays put and works offline. If a cover is wrong or won't load, open the record and either paste a URL or hit Upload… to pick an image from your computer — that always works. Click Auto-save to file once so everything persists. If covers stay blank, your browser or an extension is blocking Apple's service — upload them, or host the file (ask me how).
